Senior Community Health Navigator

Empowered Communities Opioid Project (ECOP)

The Empowered Communities Opioid Project (ECOP) is a regional collaborative that will partner the Prince William Health Department, George Mason University, the Department of Behavioral Health Services, Community Services Boards, the Virginia Correctional System, Mason and Partners Clinic, Criminal Justice Services, Local Offender Probation Services and Skill Resources. This ECOP will create a unique and innovative approach to addressing the opioid crisis by adding needed medical and social services components to care coordination for at risk released inmates from the regional jail system. Expectations and Responsibilities:

The Community Health Navigator Lead (CHNL) will be responsible for coordinating the other CHN’s and ensuring training and scheduling of the CHN team. The Community Health Navigator Lead will dedicate 100% of his/her time to the position. Additionally, he/she will assist released inmates and their families to navigate and access community services, other resources, and adopt healthy behaviors. The CHN supports providers through an integrated approach to care management and community outreach. As a priority, activity will promote, maintain, and improve the health of the released inmates. Provide social support and informal counseling, advocate for individual and community health and social needs. Additionally, the CHNL will support the program manager and provide program administrative support; assist in the program administrative aspects of implementing this grant including logistical set-up of meetings and trainings; mailing of resources; and other general administrative duties. This position will be a member of the research project team and will enter or update data into a computer system. This person will be responsible for collecting and inputting the data for the grant outcomes, putting them into a database and ensuring the evaluation team at VDH has the needed information. Additionally, he/she will assist the project manager in the coordination of inmate/patient needs.

Required Qualifications: Good communication skills; strong leadership ability; ability to manage people and delegate work and hold individuals accountable to outcomes; strong organization skills; and an attention to detail.

Preferred Qualifications: Candidate will have earned a Bachelor’s degree, or an equivalent combination of education and experience. Demonstrated program management experience.
